Staying in Place

The Pemberton CRC has partnered with Home Made and Mable to support the seniors of Pemberton to Stay in Place.

Launching in 2022, the Staying in Place is a project designed to assist older individuals in accessing government funding to provide services to support them at home.

Do you need help with any of the following?

Mobility equipment - Garden maintenance

Travel and transport - Food preparation

Social support - Domestic & cleaning support

Personal care and more - Care Management

We can help you:

  • Login and/or create My Gov account

  • Register with My Aged CareAccess

  • Online/phone assessments

  • Get your AC Number

  • Support with enquiries

  • Advocate for support

  • Book your assessments

  • Find other support services

  • Keep you informed and up to date

Eligibility

You must be over 65 years. If you are Indigenous you are eligible from the age of 50+.

You may be eligible for these and other services through a home care package.

Getting your package

First Step

Register with My Aged Care and take down your AC Number

Online/phone Assessment

My Aged Care will do an assessment that will take from 1 to 3 hours

Assessment outcome

You'll receive a letter to tell you if you are eligible for the package and at what level

Package Allocation

If successful in your package allocation, there is a 6-12 month waiting period for your referral code.

Referral Code

Once you receive your referral code, you can choose us to assist you in getting your support. This determines the level of support you need (High, Medium or Standard)
